Family Court 


The Director and Registrar of Births, Deaths and Marriages Victoria, in consultation with the Family Court of Australia and Federal Circuit Court of Australia, has requested that if a change of name of a child has been ordered by the Court and is to be registered in accordance with s 28(2) of the Births Deaths and Marriages Registration Act 1996 (Vic) the following wording in respect of such orders is preferred:

Order 1:
The child previously known as [Given name Middle name SURNAME], born on [dd/mm/yyyy], now be known as [Given name Middle name SURNAME] (eg John Mark CITIZEN) or [Given name Middle name SURNAME-SURNAME] (eg John Mark CITIZEN-SMITH) or [Given name Middle name SURNAME SURNAME] (eg John Mark CITIZEN SMITH).

Order 2:
The [Applicant/Respondent] apply to the Victorian Registry of Births Deaths and Marriages to register the change of the child’s name, in accordance with [Order 1], and do all such acts and things and sign all such documents as may be required to give effect to that registration.
